Directions for Use

Place the chart on a wall or easel 10 feet away from the subject.

Cover one eye completely using a hand, large spoon, or another item (do not apply pressure).

Identify a line that can be read comfortably and read the letters aloud. Have an assistant record your accuracy.

Continue reading letters on successively smaller lines without squinting.

Stop when less than 50% of the letters on a line are correctly identified.

Switch to the other eye and repeat the test.
